- add --layout-page-size=N option, default to --layout-page-size=32768 on AMD Bulldozer
- fix cxx3.sh for ppc
- support for arm
- handle various DW_OP_GNU parameters
- support copying over extended attributes
- fix handling of DW_OP_call_ref
