Fix Ubuntu 20.10 build and few changelog warnings
`debtransform` creates source page in non-native Debian
format, which requires Debian version prefix. Version without
prefix became a fatal error in recent `dpkg-source` versions.
I added prefix manually, but ideally it should be added by
`debtransform` automatically.

Fix Ubuntu 20.10 build
This also fixes a few `debian/changelog` warnings and
adds link to upstream repo.
The fix for Ubuntu 20.10 adds debian version prefix.
Because `debtransform` creates non-native source package,
the debian version is obligatory, and it became an error
in newest `dpkg-source`.
I submitted the bug upstream to fix this.
